The timing of the biopsy
Neoplasias
When there are neoplasias, a biopsy should
always be performed if malignancy cannot be ruled out with
confidence. Exceptions should be made only when many lesions
are present and a lengthy growth period is already known for
the malign neoplasias that are at issue in differential diagnostics,
for example in cases of multiple solar keratoses or striking
pigment-cell tumours. In many patients, the neoplasias are
so numerous that a biopsy of all suspicious-looking lesions
is impracticable. Since initial squamous epithelium carcinomas
and melanomas generally show intra-epithelial growth lasting
over a month to a year, in such case clinical progress-checks
are justifiable.
